RPM Episode Workflow:

Below are the elements required to build an RPM workflow based on an episode of care. Encounters - RPM Vitals and/or patient interactions - are tied to the “RPM Episode”.

1. Provider Referral & RPM Order

Include:

• Reason for RPM
• Devices Required
• Length of time for monitoring (30-60-90 days)

2. RPM Nurse Communication of new referral

   Potential workflow(s):

1. New inbasket message (shared pool)

   

2. New patient referral noted on RPM report from EHR report library

   

3. Patient Onboarding

1. Create an “episode” for RPM, all encounters for RPM will fall under the episode
2. Consent workflow - determine method for consenting patient and storing signed documents
3. Hub association with patient in demographics area

c. Patient Generated Data Flowsheet

d. Patient Episodes / Outreach

4. Patient Graduation from Program

• Must have process to disassociate patient from Hub (from demographic area)

5. Revenue Cycle - if billing for RPM

• Must collect time and/or 16 vitals per month
