The organization will need to determine the process for patients to receive the Stel Vitals Hub and appropriate devices:

• Central distribution, decentralized or outsourced
  • Options for distribution may include:
    • Clinic - device and Hub kits available in the clinic and provided to patients prior to visit end
    • Emergency Department (ED) - device and Hub kits available in the ED and deployed to patient prior to discharge
    • Inpatient Setting - device and Hub kits available on a particular unit and deployed to patient prior to discharge
    • Home - device and Hub kits mailed to patient’s home or delivered by community or home health team member
• Will patients keep devices or return?
• Will patients keep hub or return?

If internal:

• Storage location / work space for RPM program and kit distribution
• Par inventory levels for Hubs and devices, and inventory management processes
• Notification process to initiate kit distribution
• Return process for Hub and/or devices (mailer, returned at clinic appointment, keep for future use, pick up by community or home care team)
  • policy around unreturned equipment and retrieval attempts
• Establish a cleaning process for returned Hubs and devices

If external:

• Develop communication workflows with selected distribution service vendor
