If RPM is new to the organization, decisions around the following areas will need to be determined:

Identification of patient and consenting process:

1. Is there an existing Chronic Care Management (CCM) or RPM program?

• Are there portions of these programs that will be replicated in the Stel RPM program?

  • How are patients identified? (order/provider referral)

  • Is the team currently using a platform vendor?

• Is there a consideration for integration into the vendor’s platform?
• Has a patient consenting process been established? Can this be leveraged for the Stel RPM program or will a new process need to be developed?

1. What will be the entry point for the Stel RPM Program - Clinic, Inpatient, ED, Home, Other?
2. Will the data be directed to an external platform, the EHR or both?

Clinical Interventions

• Establish thresholds for clinical intervention
• Escalation protocols
• Expectation for sharing/disseminating patient data to patient's physician
• Patient inbound communication
• Identify first responders for patient questions and troubleshooting
• Tracking system for patient communication
• Protocols for patient follow up established
