Receiving Vitals Data from the Stel Hub: Endpoint(s):

The organization’s interface team will need to establish a secure endpoint to accept the vitals data from the Stel Vitals Hub

• In order to receive these measurements, Stel’s clients typically expose a JSON-based HTTPS endpoint.
• Refer to the Stel Integration Guide document for instructions on developing these endpoints and testing / receiving data.

RPM Order / Referral:

• Begin with the end in mind - if the program is expected to grow, consider multiple types of remote patient monitoring in the build:
  • Blood Pressure
  • Glucose
  • Heart Rate
  • Respiratory Rate
  • Weight
  • Other
• Also Consider
  • Parameters for upper and lower values for vitals transmitted
  • Escalation pathways for out of bound vitals
  • Dashboard development for clinical teams
  • Discontinuation process for RPM

Stel Vitals Hub Association in EHR or Platform

Consider workflow of where the Hub will be delivered to patients (in clinic / ED / Inpatient setting or delivered to patient home)

• Leverage bar code scanning technology if possible to associate the Hub with patient identification
• The team associating the Stel Vitals Hub in the patient record (typically the patient identification management area) may require a change to their provisioning in order to access this area of the EHR

Receiving Data: Patient Generated Data (PGD) Flowsheet

To accept vitals data from the Stel Vitals Hub, a PGD flowsheet will need to be developed based on the specifications of the EHR vendor. Please see High Level Wire Frames for examples.

Patient Triage / Clinical Dashboards

As the program scales, there will be a need to develop a process to quickly identify patients whose vitals may need review.

This may be a dashboard which highlights:

• Patients on RPM (may be sorted by program)
• Vitals not taken as expected
• Vitals out of range
• Vitals within range, positive trends, patient reinforcement outreach
• Order expiration or renewal

Completion of RPM

When a patient graduates from RPM, you will need a process to track
